Patient Rights & Responsibilities

As a patient at Journey Orthotics & Prosthetics, you have the right to:
Respect & Dignity: Be treated with courtesy, dignity, and respect regardless of age, race, gender, religion, national origin, sexual orientation, or disability.
Privacy & Confidentiality: Expect that all communications and records related to your care will be treated as confidential.
Informed Consent: Receive clear and complete information about your diagnosis, proposed treatment, alternative risk, and prognosis before consenting to any procedure.
Participation in Care: Be actively involved in decisions regarding your care and treatment.
Refuse Treatment: Refuse any treatment or service to the extent permitted by law and be informed of the consequences of such refusal.
Access to Information: Review and request copies of your medical records, in accordance with applicable law.
Quality Care: Receive appropriate and high-quality care without discrimination.
Voice Complaints: Voice concerns or complaints about your care and receive a timely response without fear of retribution.
Continuity of Care: Be informed of follow-up care and be referred to another provider when appropriate.

The products and/or services provided to you by supplier legal business name or DBA are subject to the supplier standards contained in the Federal regulations shown at 42 Code of Federal Regulations Section 424.57(c). These standards concern business professional and operational matters (e.g., honoring warranties and hours of operation). The full text of these standards can be obtained from the U.S. Government Printing Office website. Upon request we will furnish you a written copy of the standards.
As a patient at Journey Orthotics & Prosthetics, you are expected to:
Provide Accurate Information: Provide accurate and complete information about your health, medical history, and other relevant details.
Follow the Treatment Plan: Follow the care and treatment plan developed for you.
Respect Appointments: Keep scheduled appointments and notify the clinic in advance if you are unable to attend.
Be Considerate: Treat Staff, other patients, and visitors with courtesy and respect.
Financial Obligations: Meet your financial obligations by paying for services as agreed.
